Skilled Nursing Facility


The Skilled Nursing Unit Facility is a 12-bed unit dedicated to patients who require an extended period of time in order to meet their full physical potential. This specialized unit is located within the hospital and provides skilled nursing services and skilled rehabilitation services to its patients.

Criteria For Admission: Skilled nursing care must be furnished by or under the general supervision of licensed nursing personnel and the direction of a physician on the Lane Memorial Hospital Medical Staff. For a patient to be eligible:

  • The patient must require skilled nursing services or skilled rehabilitation on a daily basis, that is related to admission diagnosis or to a condition that arose during the acute stay or skilled nursing facility stay.
  • The service provided cannot be achieved at a lower level of care, such as home health or in an outpatient setting.

Team Approach To Medical Services: Our healthcare team works together to provide a full range of services. Whenever possible, we encourage the participation of family members in the care of their loved ones. Our staff will encourage you to help yourself, where possible, in order to aid in your recovery.

Medicare: Upon admission you will receive a Medicare fact sheet which lists the services covered by Medicare. If you have any questions concerning items not covered by Medicare, please ask the nurse to arrange for the unit director to visit you.

Telephone and Television: Televisions and telephones are provided by the facility. To use the telephone for local calls, dial "9", wait for a dial tone, and then dial your number.

Electrical Appliances: For your protection, electrical items such as hair dryers, electric razors, and radios must be checked by the hospital maintenance staff. Electrical extension cords are prohibited by law. Please notify your nurse to have your items checked.

Meals: Lane Skilled Nursing Facility provides three delicious meals that are nutritionally balanced. These meals are planned by the clinical nutritionist. Snacks are also provided to patients. Special diets to meet patients' needs are also available. For the convenience of your family or visitors, the cafeteria is open for breakfast from 7:00 am to 10:00 am on weekdays and 9:00 am to 10:00 am on weekends. Lunch is served from 11:15 am to 1:30 pm and supper from 5:00 pm to 6:00 pm daily. There is a snack room in the cafeteria that is open 24 hours.

Personal Items: To make your stay more comfortable, please provide personal items such as toothbrush, toothpaste, razor, shaving cream, dentures, denture cups, shampoo, comb, hair brush, mouthwash, and lotion. Patients are encouraged to wear their own pajamas, robes, and slippers. Hospital gowns are provided if needed. We request that you do not keep money, jewelry, or other valuables in your room. Valuables may be kept in the hospital's safe until your discharge from the unit. The hospital is not responsible for money or valuables.

No Smoking: Lane Memorial Hospital is a smoke-free hospital. Family members and visitors may smoke in the designated smoking area.

Visitation: We have open visitation. Children may visit at any time during the day if they are well supervised. Our Activity Room is available for larger groups. If your medical condition warrants a family member spending the night in your room, please get prior approval from the Unit Director or his designee.

Sitter Service: Should you desire, you may make arrangements for a sitter. You will be responsible for paying the sitter. Please let our staff know if a family member or any sitter will be staying in the patient's room.

Discharge: Your physician may order home health care for you after discharge. Lane Home Health is available to help you with your medical needs after you are discharged from the unit, if you desire to choose our agency.
